Registration Paperwork Turn-In

We will be inside the portable on June 5th from 6-8pm to collect any missing registration items. If you choose to wait until a night of conditioning to turn in these items, your athlete will not be allowed to participate until the next scheduled conditioning day. Below is a direct link to all the forms you will need. Two forms require notary signature. We will have one on-site free of charge. Do not sign or date those two forms until you arrive & in witness of a notary. BRING YOUR PHOTO ID!

1. Athlete's Photo (Taken by EBB)
2. TCYFCC Registration Form (notary)
3. Original Birth Certificate (new athletes only)
4. TCYFCC Liability Waiver (notary)
5. TCYFCC Concussion Form
6. Florida High School Physical (exam completed in 2023)
7. Balance paid in full
