Below are forms that will be needed to register your child for football and/or cheerleading for the upcoming 2025 season.
The Florida High School Sports Physical should be printed out and given to your child's Doctor or Medical center where their physical will be performed. All physicals must be on this form ONLY and dated for the same year as the enrolled season.
The other three forms should be filled out and brought into our facility during designated dates and times to be communicated before the season begins in which designated Board members will review, make copies as well as notarize if needed. PLEASE remember that it is imperative to bring your driver's license. If your child is NOT a returning player from last year's 2024 Season, you will need to bring your child's original birth certificate.
